How to file a claim and receive your replacement device.

Visit your membership portal and select the applicable device. Once filing is complete, you will receive a confirmation email.

Keep an eye out for delivery and wait for your replacement device to arrive.

Remove SIM card from the old device and insert into the new device. If possible, reset the old device to factory settings before shipping.4

    Preferred Care service contract is provided by Asurion Warranty Protection Services, LLC, or one of its affiliates.

    Select devices that only have a cracked front screen are eligible for cracked screen repair (devices with a damage to the back glass or other damaged components beyond front glass are not eligible for cracked screen repair). Subject to parts availability, and available in select locations, which are subject to change at any time. Repairs are performed by an Asurion-certified technician and come with a 90-day limited warranty. Eligibility for scratch screen repair will be determined at the time of claim filing.
